Official Rules

1. Sponsor

SJF Castle Rock Cryo Care, LLC, d/b/a Castle Rock Regenerative Healthcare, 4348 Woodlands Blvd, Ste 135, Castle Rock, CO 80104.

2. Eligibility

Open to legal residents of Colorado 18+ as of the entry date. Employees, contractors, and immediate family/household members of Sponsor are not eligible. Void where prohibited.

3. Entry Period

From June 17, 2026 until check-in closes at the Event on July 9, 2026.

4. How to Enter

(a) RSVP at https://castlerockregenerativehealth.com/grand-opening/ or 303-663-6990;

(b) attend in person and complete a guided sign-in at BOTH Sponsor locations — the headquarters and the Longevity Lab — obtaining both initials on the official passport. Limit one entry per person. All steps required; a partial passport is not a valid entry.

5. Drawing

Random drawing from valid (both-initialed) passports at approximately 7:15 PM on July 9, 2026. Entrants MUST BE PRESENT TO WIN. If a drawn entrant is absent or does not claim within a reasonable time, an alternate is drawn. One prize per person.

6. Prizes & ARV

(1) Core Concierge Longevity Membership — first three (3) months’ fees waived, ARV ~$2,985; winner must enroll in a Core Concierge membership within seven (7) days of notification or forfeit.

(2) Longevity Lab Pro membership at 50% off the monthly rate for 12 months, ARV ~$4,170.

(3) Longevity Lab Member membership, fees waived for 12 months, ARV ~$3,540. Total ARV ~$10,695.

7. Prize Conditions

Each prize is a membership-fee benefit only and is subject to Sponsor’s standard intake, eligibility, and clinical screening; no prize guarantees any specific service, treatment, or result. Prizes are non-transferable, have no cash value, and may not be substituted except by Sponsor. LL prizes must be activated within 7 days of winning.

8. Taxes

Prizes may be taxable income. Each winner is solely responsible for all taxes and must complete an IRS Form W-9 to claim; Sponsor will issue an IRS Form 1099 where required. Failure to provide a completed W-9 forfeits the prize.

9. Winner Obligations

Winner must sign a Prize Acknowledgment and Publicity/Liability Release and complete a W-9 at the Event, and consents to use of name, likeness, and photos for Sponsor marketing without further compensation, except where prohibited.

10. Limitation of Liability

By entering, entrants release Sponsor from claims arising from participation or prize use, to the fullest extent permitted by law.

11. Privacy

Information collected administers the giveaway and supports Sponsor marketing per Sponsor’s privacy policy at https://castlerockregenerativehealth.com/privacy-policy/. Entrants may opt out anytime.

12. General

Sponsor may modify, suspend, or cancel if the giveaway cannot run as planned. Governing law: Colorado. In no way sponsored or endorsed by KLZ 560 AM or any radio partner.
